- MBBS Degree Certificate and marksheets
- Postgraduate Degree Certificate (MS/MD/DNB) and marksheets
- Medical Council Registration Certificate (MCI / State Medical Council)
- Experience Certificates from all previous employers (on official letterhead, with period, designation, pay — appointment order NOT accepted as experience proof)
- If marks are in Grade format: Certificate from University showing Grade-to-Percentage conversion
- Self-attested photocopies of all the above documents
- Passport-size photographs

Important Notes
- Report by 9:00 AM — registration closes at 10:00 AM sharp; no late entries
- Contract period: 11 months — after completion, renewed with a 1-day break for up to 11 more months as per departmental requirement
- Selected candidates may be transferred to any health facility under AMC — prior consent required
- Any political or institutional pressure at any stage will lead to immediate cancellation of appointment
- Experience will be verified from original experience certificates on official letterhead — appointment orders alone are NOT accepted as experience proof
- If CGPA/Grade, University conversion certificate mandatory; else minimum marks will be considered
- AMC reserves the right to modify, cancel or change the recruitment process at any stage without giving reasons
